
Juliette Puttick

I am a registered nurse based in the South West of England. I trained at the University of the West of England in 1997 and qualified in 2000. I started my career in the Bristol Royal Infirmary working on acute medical wards and HDU/ITU.
In 2005 I moved into community nursing and began by specialising in Rapid Response, assessing acutely unwell patients and implementing any care needs they had to avoid hospital admissions. In 2007 I worked for the private sector alongside my NHS role where I cared for, and case managed, a gentleman with spinal injuries following a road traffic accident.
In 2009 I worked as a Team Leader with the District Nursing Out of Hours Team where we cared for any patients that required district nursing services overnight. This mainly involved caring for Palliative Care patients which led me to find my passion for Palliative Care.
In 2013 I joined the Palliative Care Home Support team as a Team Leader and became a Fast Track Nurse Assessor. Within this role I ran a team of nurses and HCAs caring for palliative patients in their own homes in their last weeks of life. I triaged referrals, assessed the patient’s care needs, implemented the care required and case managed these patients. Alongside this I became a Fast Track Nurse Assessor, assessing any patient who had been deemed reaching a terminal phase, for any care required. I would commission their care with the Brokerage Team and partner organisations such
as St Peters Hospice and Marie Curie, always ensuring care was cost effective and appropriate. I would case manage the patients and alter care as needs changed.
In 2019 I moved to North Somerset Community Hospital where I was ward sister for a rehabilitation unit. I ran a multidisciplinary team to ensure the unit ran well, that the patients were cared for and that they were discharged in a timely manner to ensure flow through the acute hospitals. From this role I was requested to lead the teams working in other Pathway 2 (Rehabilitation) and Pathway 3 (Assessment) bed bases and ensure flow through these beds. In these roles I frequently worked alongside the Continuing Health Care team, completing Mental Capacity Assessments, CHC checklists and supporting CHC assessments.
In 2022 I became Operational Lead for the Community Transfer of Care Hub which is where I am currently working. I operationally lead the Bristol and the North Somerset teams, triaging referrals, assessing patients care needs to be able to establish the care package/placement they would need to be able to be discharged from an acute hospital setting.
